    Notes for Consumers: Do not take Grapefruit juice or grapefruit-containing foods and Selpercatinib together without talking to your health care provider. Side effects from Selpercatinib may get worse if you take these together, including the risk of a dangerous change in heartbeat or heart rhythm. Contact your health care provider immediately if you experience chest pain, dizziness, fainting or falling spells, palpitations, shortness of breath, or a change in your heartbeat (such as a fast or irregular heartbeat), high blood pressure, or signs and symptoms of liver injury like dark yellow or brown urine; general ill feeling or flu-like symptoms; light-colored stools; loss of appetite; nausea; right upper belly pain; unusually weak or tired; yellowing of the eyes or skin.
    Notes for Professionals: Advise patients to avoid coadministration of selpercatinib and grapefruit juice due to the risk of increased selpercatinib exposure which may increase the risk of adverse reactions, including QT prolongation. Selpercatinib is a CYP3A4 substrate and grapefruit juice is a strong CYP3A4 inhibitor. Coadministration with another strong CYP3A4 inhibitor increased selpercatinib exposure by 133%.
