AMIODARONE (a MEE oh da rone) prevents and treats a fast or irregular heartbeat (arrhythmia). It works by slowing down overactive electric signals in the heart, which stabilizes your heart rhythm. It belongs to a group of medications called antiarrhythmics.

Caffeine-containing Foods/Beverages
· Severity: Mild
· Notes for Consumers: Side effects from Caffeine may get worse if you take Amiodarone. Too much Caffeine can cause effects like nausea, nervousness, or sleeplessness. You may need to limit your Caffeine intake. Common foods and beverages containing caffeine include coffee, teas, colas and chocolate. Many non-prescription drugs and diet aids may also contain caffeine.
· Notes for Professionals: Amiodarone is an inhibitor of CYP1A2 isoenzymes, and could theoretically reduce CYP1A2-mediated caffeine metabolism. The clinical significance of this potential interaction is not known.

Marijuana
· Severity: Moderate
· Notes for Consumers: The effects of marijuana may be increased and side effects may get worse if it is combined with this medicine. Do not drive or operate machinery until you know how this combination will affect you. Contact your health care provider right away if you notice slurred speech, confusion, severe drowsiness, increased heart rate, or any other new or unusual side effects.
· Notes for Professionals: The incidence of marijuana associated adverse effects may change following coadministration with amiodarone. Amiodarone is an inhibitor of CYP2C9 and CYP3A4, two isoenzymes responsible for the metabolism of marijuana's most psychoactive compound, delta-9-tetrahydrocannabinol (Delta-9-THC). When given concurrently with amiodarone, the amount of Delta-9-THC converted to the active metabolite 11-hydroxy-delta-9-tetrahydrocannabinol (11-OH-THC) may be reduced. These changes in Delta-9-THC and 11-OH-THC plasma concentrations may result in an altered marijuana adverse event profile.

Grapefruit juice
· Severity: Major
· Notes for Consumers: Avoid eating grapefruit or drinking grapefruit juice while taking Amiodarone, unless advised differently by your healthcare professional. Grapefruit juice may increase the level of Amiodarone in the blood, by decreasing the elimination of Amiodarone from the body. Contact your health care provider immediately if you experience palpitations, chest pain, passing out or dizziness, shortness of breath, or a change in your usual heartbeat while taking Amiodarone.
· Notes for Professionals: Grapefruit juice has been shown to increase amiodarone peak serum concentrations and AUC when a single dose of amiodarone was administered orally. No change in ECG or arterial blood pressure measurements were identified in this single dose study; however, the impact on chronic dosing of amiodarone was not evaluated. To prevent potential drug accumulation, it would be prudent to avoid taking oral amiodarone with grapefruit juice.
