LOPERAMIDE (loe PER a mide) treats occasional diarrhea. It works by slowing down an overactive bowel, which decreases the number of bowel movements. It belongs to a group of medications called antidiarrheals. It is often used for a short period of time.

    • Severity: Moderate
    • Notes for Consumers: Although an interaction is possible, these drugs may be taken together with caution. Taking these drugs together may increase the risk of a dangerous change in heartbeat or heart rhythm. Contact your prescriber immediately if you experience chest pain, dizziness, fainting or falling spells, palpitations, shortness of breath, or a change in your heart beat (such as a fast or irregular heart beat). Also, you may feel drowsy or may have trouble breathing when taking these drugs together. Do not drive or operate machinery until you know how these drugs affect you. If you have difficulty staying awake or notice slurred speech, confusion, severe weakness, or difficulty breathing, contact your health care provider immediately.
    • Notes for Professionals: The plasma concentration of loperamide, a CYP3A4, CYP2D6, and P-glycoprotein (P-gp) substrate, may be increased when administered concurrently with grapefruit juice, an inhibitor of CYP3A4, CYP2D6, and P-gp. If these drugs are used together, monitor for loperamide-associated adverse reactions, such as CNS effects and cardiac toxicities (i.e., syncope, ventricular tachycardia, QT prolongation, torsade de pointes, cardiac arrest).
