Ixabepilone 15mg, Lyophilisate for solution for injection
• Interaction: Grapefruit juice• Severity: Severe• Notes for Consumers: Do not take Ixabepilone with grapefruit or grapefruit juice. Taking Ixabepilone with grapefruit or grapefruit juice may increase side effects of Ixabepilone. If you accidentally take Ixabepilone with grapefruit or grapefruit juice, immediately tell your doctor about any unusual effects.• Notes for Professionals: Ixabepilone is a CYP3A4 substrate, and grapefruit juice inhibits the cytochrome P-450 3A4 isozyme in the gut wall. Grapefruit juice contains furanocoumarins that are metabolized by CYP3A4 to reactive intermediates. These intermediates form a covalent bond to the active site of the CYP3A4 enzyme, causing irreversible inactivation (mechanism-based inhibition). Consequently, CYP3A4 activity in the gut wall is inhibited until de novo synthesis returns the enzyme to its previous level. In addition, ixabepilone is a P-glycoprotein (Pgp) substrate and grapefruit juice is a Pgp inhibitor. Grapefruit juice may increase ixabepilone concentrations and should be avoided. -

• Interaction: Alcohol• Severity: Major• Notes for Consumers: Ixabepilone contains a large amount of alcohol. Avoid Alcohol-containing products or beverages on the days you get Ixabelilone, as excessive sedation (difficulty staying alert), dizziness, or difficulty breathing may occur.• Notes for Professionals: The supplied diluent that must be used for reconstitution of ixabepilone has a high concentration of dehydrated alcohol (39.8% w/v). Additive drowsiness may occur if a patient drinks ethanol immediately before or after getting ixabepilone.
