Marijuana

· Severity: Moderate

· Notes for Consumers: The effects of marijuana may be increased and side effects may get worse if it is combined with this medicine. Do not drive or operate machinery until you know how this combination will affect you. Contact your health care provider right away if you notice slurred speech, confusion, severe drowsiness, increased heart rate, or any other new or unusual side effects.

· Notes for Professionals: The incidence of marijuana associated adverse effects may change following coadministration with aprepitant, fosaprepitant. Aprepitant, fosaprepitant is an inducer of CYP2C9 as well as an inducer and inhibitor of CYP3A4, two isoenzymes responsible for the metabolism of marijuana's most psychoactive compound, delta-9-tetrahydrocannabinol (Delta-9-THC). When given concurrently with aprepitant, fosaprepitant, the amount of Delta-9-THC converted to the active metabolite 11-hydroxy-delta-9-tetrahydrocannabinol (11-OH-THC) may be altered. These changes in Delta-9-THC and 11-OH-THC plasma concentrations may result in an altered marijuana adverse event profile.

Grapefruit juice

· Severity: Major

· Notes for Consumers: Do not take Aprepitant, Fosaprepitant with grapefruit juice. Increased side effects may be observed if these drugs are given together. Discuss possible alternative options with your health care provider.

· Notes for Professionals: Avoid the concomitant use of aprepitant, fosaprepitant with grapefruit juice due to substantially increased exposure of aprepitant. After administration, fosaprepitant is rapidly converted to aprepitant. Grapefruit juice is a strong CYP3A4 inhibitor and aprepitant is a CYP3A4 substrate. Coadministration of a single oral dose of aprepitant on day 5 of a 10-day ketoconazole regimen (strong CYP3A4 inhibitor) increased the aprepitant AUC approximately 5-fold, and increased the mean terminal half-life by approximately 3-fold.
