SUFENTANIL (soo FEN ta nil) treats severe pain. It is prescribed when other pain medications have not worked or cannot be tolerated. It works by blocking pain signals in the brain. It belongs to a group of medications called opioids.
Sufentanil 30mcg, Sublingual tablet
Grapefruit juice
· Severity: Moderate
· Notes for Consumers: Although an interaction is possible, Sufentanil is only used in a hospital or clinic. You may feel drowsy or more tired when taking this combination together. Do not drive or operate machinery until you know how the combination affects you. Discuss any new or worsening side effects, such as breathing problems, confusion, constipation, dizziness, trouble passing urine or change in the amount of urine, dry mouth and eyes, changes in vision, or fast heartbeat with your health care provider.
· Notes for Professionals: Because the dose of the sufentanil sublingual tablets cannot be titrated, consider an alternate opiate if grapefruit juice is regulary consumed. Consider a reduced dose of sufentanil injection with frequent monitoring for respiratory depression and sedation if patient consumes grapefruit juice. If grapefruit juice intake is significantly reduced or stopped, consider increasing the sufentanil injection dose until stable drug effects are achieved and monitor for evidence of opioid withdrawal. Sufentanil is a CYP3A4 substrate, and coadministration with a strong CYP3A4 inhibitor like grapefruit juice can increase sufentanil exposure resulting in increased or prolonged opioid effects including fatal respiratory depression, particularly when an inhibitor is added to a stable dose of sufentanil. If grapefruit juice intake is significantly reduced or stopped, sufentanil plasma concentrations will decrease resulting in reduced efficacy of the opioid and potential withdrawal syndrome in a patient who has developed physical dependence to sufentanil.

Alcohol
· Severity: Major
· Notes for Consumers: Avoid alcohol-containing drinks while taking this medicine. Using this drug along with alcohol can lead to serious side effects. You may feel drowsy or more tired when taking these drugs together. Do not drive or operate machinery until you know how this drug affects you, and especially if you are drinking alcohol-containing beverages. If you notice slurred speech, confusion, severe weakness, or difficulty breathing, contact your health care provider immediately.
· Notes for Professionals: Alcohol is associated with CNS depression. The combined use of alcohol and CNS depressants can lead to additive CNS depression, which could be dangerous in tasks requiring mental alertness and fatal in overdose. Alcohol taken with other CNS depressants can lead to additive respiratory depression, hypotension, profound sedation, or coma. Consider the patient's use of alcohol or illicit drugs when prescribing CNS depressant medications. In many cases, the patient should receive a lower dose of the CNS depressant initially if the patient is not likely to be compliant with avoiding alcohol.
